What is skin aging?

Skin aging is the gradual accumulation of cellular and structural damage in the skin. Over time, DNA injury, oxidative stress, inflammation, and glycation weaken collagen and elastin, slow cell turnover, impair barrier function, and lead to wrinkles, laxity, dryness, uneven tone, and enlarged pores.

Two types of aging interact:

- Intrinsic (genetic/chronologic): Changes driven by genetics and time—thinner dermis, slower repair, reduced collagen and sebum.

- Extrinsic (environmental): Changes accelerated by external factors—UV and HEV light, pollution, smoking/vaping, chronic stress, poor sleep, suboptimal diet, alcohol, and inappropriate cosmetic or medical treatments.

When to start

Preventive care ideally begins in childhood with sun protection and should be routine by the mid-20s. From about age 25, collagen loss and slower repair make consistent care especially important.

Core pillars of prevention and treatment

1) Protect (daily, year-round)

- Broad-spectrum sunscreen SPF 30+ every morning (50+ for high UV), reapply every 2 hours outdoors

- Sun-protective clothing, hats, sunglasses; seek shade 10 a.m.–4 p.m.

- Consider tinted mineral sunscreen to block visible light that can worsen pigmentation

2) Repair and stimulate

- Retinoids at night (retinol): start 2–3 nights/week, then nightly as tolerated.

3) Restore and maintain the barrier

- Moisturisers with ceramides, cholesterol, fatty acids, glycerin, and/or hyaluronic acid.

- Adjust richness seasonally and for skin type.

4) Lifestyle medicine

- Do not smoke or vape; limit alcohol

- Prioritize sleep (7–9 hours), exercise regularly, and manage stress

- Eat a Mediterranean-style diet: colorful produce, lean protein, omega-3s; reduce added sugars to limit glycation

- Stay hydrated; protect skin from irritants; patch-test new products
